How this website handles the information you send us. Your medical records are covered separately by our Notice of Privacy Practices, available at the clinic.
What we collect
When you send the visit request form, we receive your name, phone number and anything else you choose to add, such as your email, the date of the accident and where it hurts. We also receive the page you sent it from and, if you arrived from an ad, the campaign details in the link.
How we use it
Only to contact you about your visit. We do not sell your information or share it for others' marketing.
Please don't send medical details here
The form is for scheduling. Share your medical history at your visit, where it is protected under HIPAA.
Cookies and ads
This site may use Google and Meta advertising and analytics tags to measure visits and ad results. They don't receive what you type in the form. You can block them in your browser settings.
Calls and texts
If you check the consent box, we may call or text the number you gave about your request. Reply STOP to any text to opt out.
